Apply for boxes

One of the Daniel Estick Trusts objectives is to provide Memory Boxes to Bereaved Parents and Families following the loss of a child. To achieve this, we provide boxes ready for distribution to recognised support services who offer face to face support to bereaved families.

If you would like to apply to receive a donation of Boxes from the trust, please complete and submit the form below.

  • Please note this cannot be a personal address, it should be the registered address of the organisation you are applying on behalf of.
  • Please provide name and contact details if different from those entered above.
  • Please provide us with a brief outline of how and you intend to use the boxes within your organisation, including how you will control the distribution of boxes within the organisation to ensure they are only used to fulfil the objectives of the trust.
  • As we are keen to provide boxes where required across the whole of the UK we are often unable to deliver consignments in person. We fundraise hard to make the boxes and provide them to parents free of charge. We do ask for a small contribution towards postage and packaging costs allowing us to use secure tracked courier services to deliver units to organisations that are unable to collect boxes directly from the trust in Milton Keynes.

Please indicate below the number of boxes you require and confirm that you are happy to contribute towards the associated P&P. If you’re application is approved we will send you a discount code to apply in our online shop making the boxes you order free of charge leaving just the postage to pay .
  • We are using reCAPTCHA to check that you a real person. Please check the box to proceed.

Thank you for helping the trustees of The Daniel Estick Trust realise our vision that all bereaved parents should have access to a memory box and support when they need it the most. Something the parents of Daniel didn’t have when they were faced with leaving their local hospital empty handed after Daniel’s sudden passing. We really appreciate the amazing support work other professionals and organisation can offer parents who find themselves in this tragic situation.
